December 2023 year-end assessments are now due. Please complete this, and any other outstanding year-end assessments, as soon as possible.

Registration of new patients, Transfers, End of Treatment entries and Transplant Procedure entries should be completed as these events occur.

Only authorised personnel may access the Registry, logging in with the username and password provided.

For security, an audit trail of all activity is recorded. This includes the IP address of the computer used to gain access.
